Summary of Position:
The Registered Nurse (RN) works with all members of the Patient-Centered Medical Home (PCMH) Care Team by performing a variety of clinical and administrative patient-related duties as established in the roles and responsibilities defined belowThe RN Actively participates in team huddles and care team meetingsThe RN provides professional nursing care, that is safe and effective, utilizing established policies and proceduresThe RN assesses, plans, implements and evaluates care in collaboration with physicians and practitionersThe RN provides leadership and supervision to all clinical staffThe RN coordinates department activities, ensuring safety of patients and staff and ensures that adequate medical supplies, medication and vaccinations are ordered and are on the premisesServices are provided based on the ages of the population served, newborn through geriatric (65 years or older), in collaboration with the care team.QualificationsEducation:
? Must be a graduate from an accredited school of nursing with a current Registered Nurse license specific to the State of employment? Bachelor's degree in Nursing (BSN) preferred.
Experience:
? At least two years of nursing work experience.? Ambulatory care/physician office experience preferred.? Prior experience with chronic disease management and PCMH preferred.? If applying for UC/Occupation Health Serviceso Emergency room, or Urgent Care experience is preferred
Knowledge/Skills/Abilities:
? Duties require intensive knowledge of a highly specialized field or broad knowledge of major hospital and PCMH functions or activities.? Extensive knowledge of epidemiological principles, infectious / communicable diseases, health assessment, health management, and adult immunizations.? Highly effective written and verbal communications skills.? Excellent interpersonal skills.? Proficient computer skills as required to document, communicate and enter information into the electronic medical records system.? In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.Certifications/Licensure:
? Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.? If applying for UC/OHS position a current ACLS card will be required or must be able to complete an approved outside course, or the first available Barton University certification course.? Valid state issued driver's licenseWill use own vehicle to travel to off-site locations; need proof of car insurance and registration.? Current California Nursing License in good standing for California position.? aReview of the patient history,bCompile all pertinent medical data on the employee's injury,cReview functional capacity evaluations,dDocument and submit reports of these evaluations to various client companies with a summary of findings.25Networks with client company representatives to provide resources information on all aspects of the OHS programInteracts with client company injured employees to ensure the employee has been provided the highest quality of service while a clientAssesses the educational needs of company representatives and propose programs to meet those needs.26Visits client company on a regular basis to keep current on the specific job tasks the employees are doing, provide periodic feedback beyond the patient reports of trends or just company statusDrives to client companies, or other departments to administer vaccination, perform screenings, and other duties as assigned.27Act as resource to all physicians who may interact with the injured employee to ensure appropriate knowledge of the work site and TAW/TWA (temporary alternative work/temporary work assignment) status of the client company.28Collaborates with clinical peers to evaluate current practices and ensures that the most efficient avenue is being pursued to accomplish the job.29Promotes wellness concept to all clients and to participate in health and wellness fairs.30Responds to the needs of the department by performing other duties, as necessary. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
